Blood Tests for Arthritis: RF, Anti-CCP, ESR, CRP and HLA-B27 Explained

Short answer: No single blood test can diagnose arthritis. Doctors combine your symptoms, an examination of your joints and blood tests, sometimes with imaging. The main tests are RF and anti-CCP (rheumatoid arthritis), ESR and CRP (inflammation), HLA-B27 (spondyloarthritis) and uric acid (gout). A normal result does not always rule arthritis out.

The main arthritis blood tests

TestWhat it looks forUseful for
RF (rheumatoid factor)An antibody found in many people with rheumatoid arthritisRheumatoid arthritis
Anti-CCPAn antibody that is more specific to rheumatoid arthritisRheumatoid arthritis, outlook
ESR and CRPGeneral markers of inflammationShowing inflammation, tracking treatment
HLA-B27A gene variant linked with spondyloarthritisAnkylosing spondylitis, inflammatory back pain
ANAAntibodies against cell nucleiLupus and related diseases
Uric acidUrate level in the bloodGout, monitoring treatment
CBC, liver and kidney testsBlood counts and organ functionSafety checks on arthritis medicines

RF and anti-CCP for rheumatoid arthritis

Rheumatoid factor is positive in many, but not all, people with rheumatoid arthritis, and it can also be positive in healthy older people and in other conditions. Anti-CCP is more specific, so a positive result carries more weight, and it can appear years before symptoms. Having both positive usually means more active disease that needs early treatment. About a quarter of people with rheumatoid arthritis test negative for both, which is called seronegative arthritis. Read our page on rheumatoid arthritis.

ESR and CRP: signs of inflammation

CRP rises quickly with inflammation; ESR changes more slowly and also rises with age and anaemia. Raised values show that something is inflaming the body, but not what. Infections and injuries raise them too. They are most useful for tracking disease activity after diagnosis.

HLA-B27 for back pain in younger people

Most people with ankylosing spondylitis carry HLA-B27, but so do some healthy people, so a positive result supports the diagnosis only when the symptoms fit. It is a genetic test, so it only needs to be done once. See inflammatory back pain.

Uric acid for gout

A high uric acid level raises the risk of gout, but many people with high levels never get gout, and the level can be normal during an attack. The diagnosis is confirmed by finding urate crystals in joint fluid. Uric acid is mainly used to monitor treatment. See normal uric acid ranges by age.

Why a normal blood test does not mean “no arthritis”

  • Early arthritis can have normal blood tests.
  • About one in four people with rheumatoid arthritis is seronegative.
  • Osteoarthritis and many other causes of joint pain do not show up in blood tests at all.

That is why diagnosis depends on examination and your history first. See when to see a rheumatologist.

How to prepare for the tests

Most arthritis blood tests need no fasting. Tell the laboratory about your medicines, bring your doctor’s prescription, and ask for the report to be sent to your rheumatologist. Immunix Labs offers these tests at OARC Saheed Nagar, OARC Sailashree Vihar and at home.

Frequently asked questions

Which blood test confirms rheumatoid arthritis?

None on its own. Anti-CCP and RF support the diagnosis, but doctors also rely on joint swelling, the pattern of joints affected and the duration of symptoms.

My RF is positive but I have no joint pain. Do I have arthritis?

Not necessarily. A positive RF can occur in healthy people, in older age and in some infections. A doctor needs to look at your symptoms before drawing conclusions.

Can I test for arthritis at home?

You can have blood collected at home for the tests, but the results need a doctor’s interpretation, and the diagnosis needs an examination.
